About This Opportunity
As our Health Services Advocate, you'll serve as the vital connection between participants and community healthcare providers, ensuring timely access to medical care while advocating for their needs. Based at our Libertyville campus, you'll spend your days transporting participants to appointments throughout the area, supporting them during medical visits, and coordinating their care with our nursing team.
What you'll accomplish:
- Provide advocacy and support during medical appointments, procedures, and healthcare coordination
- Build trusted relationships with community providers who deliver quality care to participants
- Create accurate health documentation that ensures seamless communication across care teams
- Advocate effectively for participants' needs during medical visits and emergency situations
What you bring to this role:
You bring experience supporting individuals who need assistance navigating healthcare systems, and you're comfortable communicating with medical professionals on someone else's behalf. Your calm presence helps people feel safe during stressful medical situations, and you understand the importance of accurate documentation in coordinating complex care.
- High school diploma or equivalent
- Valid driver's license from Indiana, Illinois, or Wisconsin with clean driving record
- Willingness to provide care involving exposure to illness while following infection control protocols
- Physical capacity to assist participants with mobility needs during appointments and transportation
